As we come to the end of our time together on this initiative, we would like to invite you all to the P-TCPi Outcomes Congress on September 20, 2019! We encourage your clinic teams to attend. In this all-day Congress, we will celebrate the success of the past four years and give you the tools needed to sustain the gains you have made as well as help practices identify and develop strategies to join an APM. Enrolled clinics are eligible for an attendance stipend of up to $3000, and free CME will be available!
