Position Description

Per diem Nurse Practitioner/Physician Assistant

Yakima, Washington

$5,000 Sign on Bonus

Role Overview:

  • Provide primary care to patients at home, nursing homes, assisted living, and senior housing
  • Deliver cost-effective, quality care to assigned members
  • Manage medical and behavioral conditions in collaboration with physicians
  • Perform comprehensive assessments and document findings
  • Ensure accurate ICD10 coding and documentation
  • Conduct medication reviews and reconciliations
  • Facilitate and implement member's plan of care
  • Coordinate care across different levels and settings
  • Promote the CCM program and maintain membership growth

Qualifications:

  • Certified Nurse Practitioner or Physician Assistant
  • Graduate of accredited programs with board certification
  • Active and unrestricted state license
  • Current DEA licensure/prescriptive authority
  • Reliable transportation
  • Ability to lift 30-pound bag and navigate various dwelling conditions
  • Availability to work 24 hours per month, including off-hours

Preferred Qualifications:

  • 1+ years of post-grad experience in Long Term Care
  • Understanding of geriatrics, chronic illness, and advanced illness
  • Proficient computer skills for medical documentation
  • Ability to develop positive customer relationships
  • Adaptability to change
